If your student has an IEP or a special need, please submit the following documentation to Emily Lewis, Special Education Lead
 

• Individualized Education Program (IEP)

• Multi-Disciplinary Team Report (MDT)

• Statement of Eligibility 

• Any outside evaluation reports/information regarding diagnoses.
